Chart problem

What causes a chart to not render, but show up only as an outline with the
little x-box in the upper left corner? I'm getting it frequently and cannot
pin down why, especially since it will work in VS, and sometimes when called
programatically and still fail under report manager.

unable to resolve the URL back to the Report Server. One cause is machine
names with underscores in them. There are sometimes other ones.
